Acutally some make them up. You can go to www.cafepress.com, register as a member and there is a whole community of designers that will help you put your ideas into a solid drawing.

is a good place to find great t-shirt designs. It's a design-by-submission company. So if you yourself have a design, you can submit it. If it gets voted on, it will be made into a t-shirt.
